Military2VA Program Supports Veterans Through the Transition to Civilian Life

Lead social worker for the Post-9/11 Military2VA program Kelly Saur appeared on Sheridan Media’s Public Pulse to inform the public about the program designed to improve access to VA health care and provide ongoing biopsychosocial support to newly separated Veterans through comprehensive transition assistance.

Every VA medical center has a specialized Military2VA team that assists service members transitioning from active duty to civilian life. The Post-9/11 Military2VA program also supports Veterans, their families and caregivers by helping them navigate the VA health care system and access services that support long-term health and wellness goals.

Transitioning back to civilian life can be challenging, even for seasoned service members. Veterans may experience difficulties related to identity, finances, employment and education barriers, relationship changes, mental and emotional health and physical limitations resulting from their time in service.

Saur said the Military2VA team is trained to address the unique needs of reintegrating Post-9/11 era Veterans.

Military2VA team members help Veterans focus on what matters most to them, from accessing care and resources to developing personalized goals for medical treatment and navigating VA benefits.

Saur said the Military2VA team also educates Veterans on VA eligibility, benefits and services, provides transition and reintegration assistance and applies a holistic approach to care planning that includes Whole Health, a personalized health plan based on individual values, needs and goals. The team also facilitates continuity of care for Veterans who relocate to another VA facility.
