The “hub” of the Hub on Smith 

Hub on Smith Support Center Manager Steve Moyer, made an appearance on Sheridan Media’s Public Pulse to discuss how the Support Center can best serve anyone looking for services.

Moyer said the center is one of the first places he recommends stopping to learn what’s available with a team that understands how to guide community members of all ages through the many services, programs and classes offered at the Hub and elsewhere.

The Support Center also has a team of care coordinators, who understand and work with the people who operate the Medicaid Community Choices Waiver program, a statewide long-term care Medicaid program for seniors and adults with disabilities who are at risk of nursing home admission.

According to the Hub, the Support Center has been designed as a comprehensive, consolidated access point within the Hub that creates a client-oriented experience. Organized to be streamlined, efficient, and to create the highest level of service delivery.

A visit to Moyer and his team can connect residents to in-home care, family caregiver support, transportation, access to meals, Hub day programs and activities, and referrals to community resources.
