Pat BlairA new partnership among the Sheridan and Montana VA Health Care System teams and two Billings hospitals will mean better healthcare for veterans.
According to a news release from the VA systems, the Sheridan and Billings VA systems have teamed with St. Vincent Healthcare and Billings Clinic in a move that’s expected to provide seamless care for enhanced discharge options through on-site VA care coordination.
According to the release, both of the partner hospitals have created permanent offices for a VA nurse. Every veteran enrolled in the Sheridan or Montana VA health care system who’s admitted to one of the partner hospitals will connect with the on-site VA nurse, and the nurse will then work closely with the respective community providers to create an integrated care plan as the veteran moves through the system.
The VA nurse will coordinate the veteran’s continued care to streamline each phase of care from admission to discharge and any continuing care such as physical therapy, follow-up appointments and prescriptions.
Approximately 50,000 veterans receive care through the Montana VA and Sheridan VA health care systems, and will be able to benefit from the new partnerships.
